Мechanism of action

Another name for isotretinoin is 13-cis-retinoic acid. It belongs to the retinoid class and is synthesized from vitamin A. How does Accutane work? Although the exact mechanism of action of isotretinoin could not be determined, several studies have shown that it causes programmable death (apoptosis) of different cells.

In the case of acne, the death of sebaceous gland cells occurs, which helps eliminate rashes. The drug also has antimicrobial effects on Cutibacterium acnes bacteria that affect the skin.

Isotretinoin is the only remedy stopping the main pathogenic processes that trigger acne development:

  • the drug reduces skin fat production
  • skin renewal is accelerated
  • it eliminates an environment conducive to bacterial reproduction
  • bacteria causing inflammation are destroyed

Due to the complex action, the drug is much more effective than antibiotics are. It allows you to get rid of severe forms of acne and is used when other treatments do not provide the desired result.


The drug is bioavailable due to its high content of isotretinoin. Bioavailability increases when the medicine is taken with food. The maximum drug concentration in the blood plasma of 80 mg was observed after 2-4 hours. At the same time, the concentration of the drug in the blood is 1.7 times lower since it does not penetrate the red blood cells.

Metabolization occurs by forming three metabolites: 4-oxo-isotretinoin, tretinoin, and 4-oxo-tretinoin. The half-life of the terminal phase is 19 hours. The drug is excreted by the kidneys and liver together with urine and bile.

Accutane dosage and intake characteristics

Accutane is available in capsules of:

  • 20mg
  • 30mg
  • 40 mg

The dosage is calculated based on the formula: 0.5-1 mg/kg of weight. Usually, it is divided into two doses per day. In the course of treatment, the dosage can be adjusted according to the results. In severe cases, it is increased to 2 mg/kg.

Since the drug is better absorbed with food, it is recommended to take pills while eating. Taking it once a day is not recommended. The capsule should be taken whole without chewing and with a full glass of water. The average treatment lasts 15 - 20 weeks or as prescribed by a doctor. Repeated treatment is possible in case of persistent rashes or relapse at least 2 months after the first course.

How does Accutane work after starting the course of treatment? In the first days, the skin condition may deteriorate. Improvement usually occurs after 7-10 days. The full drug effect can be felt after 1-2 months of treatment. At the same time, long-term administration of the drug is contraindicated.

Once taken, isotretinoin can remain in the body for about 5 days. The time of removal of the active ingredient from the body depends on the following factors:

  • dosage and dosing frequency
  • metabolic rate
  • your age and health state
  • your weight

During the treatment, you must regularly have blood tests to monitor your body condition and avoid dangerous side effects. Liver function and liver enzyme tests are recommended every 3 months. It is also necessary to monitor triglyceride levels to prevent the development of acute pancreatitis.

Side effects

The severity of Accutane side effects is highly dose-dependent. When using low doses, side effects may not manifest at all, while high dosages can trigger serious side effects. One out of 100 persons have the following complaints:

  • dry skin
  • rash, itching or redness

  • dry mouth
  • cracked and dry lips
  • nasal dryness

According to one study, when isotretinoin was taken for 2 years in daily doses of 2 mg/kg in patients with multiple basal or squamous cell cancer, the number of tumors decreased from 121 to 25, and the number of new cancers decreased by 63 per cent.
